SHIPPED SEMEN CONTRACT

This agreement is made this ______day of ________, 20______ by and between Crystal Creek Paints, Stallion Owner, Midwest Stallion Station, and :

1. A Collection and Lab fee of $150 will be charged each time the mare owner requests shipped semen.  Shipping cost of the Equitainer will be approximately $50.00 for each shipment.
2. Mare owner is responsible for the cost of returning the Equitainer.
3. Midwest Stallion Station (319-377-4091) must be notified 24 hours prior to the date and time semen is to be shipped. (Prior to 10:00am is preferred).
4. Mare owner is responsible for having a certified veterinarian experienced in  handling cooled semen inseminate the mare.
5. If the mare fails to conceive by the second insemination, a culture is required. This is to save you time and money.
6. Mare owner is responsible for pregnancy checking the mare on or before 18 days of notifying Midwest Stallion Station, Stallion owner, or said agent thereof.
7. Mare Owner or authorized representative will complete and sign contract, enclose booking fee and copy of registration papers and return them to the Stallion Owner or said agent thereof.  It will be binding on both parties subject to the above terms.
